I would work either on the ward supporting patients with their exercise plans which have either been formulated by a Physiotherapist, Occupational Therapist or an Assistant Practitioner. Within the team the AP would carry a caseload of less complex patients. Part of my role would be completing exercise plans, if the patient has mobility issues, delegating rehab support to work with the patient. Reviewing of patients, and working closely with others. As part of an Occupational role it would be formulating careplans with the patient, setting out the achievable goals, seeing if equipment which can be provided will help the patient to stay in their own home.
Within this role I was a care coordinator for the less complex patients with mental health illness, forming care plans with the patient involving those involved in that patients care however only with the patients consent.
I would need to work collaboratively with other agencies involved in the patients care. Part of the role i would complete risk assessments. I would also offer supervision for the support workers as and when required.
I was senior care within this role, part of the job would be to accept medication deliveries alongside another staff member to ensure the prescriptions are all correct and no medication missing. As part of the role it would be to ensure that the clients personal care is maintained to a high standard.
As senior I would be responsible for a safe shift ensuring clients needs are meet. Completing paper work as required.
