As a registered nurse in my unit with about 30 residents, My duty is to
maintain a person-and relationship centred approach to care,
recognising that nursing home are residents' home as well as a place
where care is delivered. Hence my role requires in-depth knowledge of
long term conditions associated with ageing, managing complex multiple
morbidities and frailty. Assisting in assessment of effectiveness of care,
nursing implementation and delivery. Implementing action to meet and
maintain care, and nursing standards. Working in cooperation with
members of the multidisciplinary team to maximise opportunities for
residents therapeutic care and ensuring residents rights are protected.

My responsibilities include but not limited to:

• Providing professional clinical nursing to all residents based on individualized care plans.
• Provide safe, person-centred nursing care to elderly residents, including those receiving end-of-life care.
• Administer medications, including controlled drugs, via various routes following NMC and care home policies.
• Monitor and assess residents' health conditions and escalate changes appropriately.
• Maintain accurate documentation including EMAR, care plan, and observations charts.
• Support residents with dignity in daily activities and promote their physical and emotional well-being.
• Collaborate with GPs, families, and ensure multidisciplinary teams to ensure continuity of care.
• Perform clinical procedures such as wound care, catheterization, venepuncture and blood sampling, Blood glucose monitoring, subcutaneous injections.
