Mental Health Practitioner - Urgent Response Team

  • This role was implemented in spring 2024 as a pilot initiative with CWP and NWAS - North West Ambulance Service. The core purpose was to specific practitioners to respond to patients in mental health distress where the need is no acute medical, to reduce ambulance attendances where not necessary and to ensure the right care assessment and pathways are accessed for patients without delay.
  • Maintained meticulous records of patients' progress for professional analysis and review.
  • Conducted risk assessments to safeguard individuals from self-harm or harm towards others.

  • The Urgent Support Team offers outreach and assessment to children and young people up to the age of 16. Duties to include the following:
    • Assessing children and young people who attend emergency departments or are admitted to a paediatric ward as part of the self harm pathway

Youth Justice Parenting Practitioner

In this role I worked with young people and their parents to support them on various orders imposed by the court this included the assessment and delivery of parenting orders imposed by the youth court. Young people involved in the service often were assessed as having complex needs to include difficult systemic relationships, parental breakdown, edge of care along with peer and substance misuse related concerns. I also worked across prevention side of the team with a caseload of parents and young people who were at risk of offending. This role required me to complete parenting assessment and court reports and delivery of 1 to 1 and group parenting interventions. I was also responsible for the planning and coordination of group interventions across Cheshire East with a focus to support both parents and young people with the challenges they had with the aim to prevent and reduce criminal/ reoffending behaviour. This role required multi agency working to include Police, Social Care, Substance Misuse services and schools to ensure coordination and support of multi agency meetings and mental health to offer robust risk management and safety planning in working towards positive outcomes for young people.
