I started my nursing carer as a community nurse in 2018 . This role enabled me to obtain many valuable and transferable skills . As a Community nurse I have extensive expertise in a wide variety of clinical skills, this includes all aspects of wound care ranging from; surgical wounds, complex leg ulcers, compression bandaging and fungating tumour wounds. I also led the doppler and leg ulcer clinics and was responsible for assessing wounds , implementing appropriate treatment plans and monitoring and reviewing care plans . I attended wound care hubs regularly to ensure my knowledge of dressings remained up to date, and also to be aware of new dressings on the Formulary. This was especially beneficial for patients with static wounds where many dressings had not been effective.
A large percentage of my caseload was caring and supporting diabetic patients, this included teaching patients to be independent in monitoring their capillary blood glucose, administering their own insulin and offering educating around appropriate dietary choices. I also carried out education sessions in Residential care homes to ensure staff felt more confident in supporting diabetic patients. Also that they were able to recognise and to know what action to take in the event of a patient becoming Hyper/hypoglycaemic . I worked collaboratively with diabetic specialist nurses in managing complex diabetic patients where insulin regimes were not effective in managing a patients CBG levels. I believe that I have grown as a nurse since working in the community and have developed excellent communication skills which has enabled me to liaise effectively with other healthcare professionals . Although this role has required me to work autonomously, I realise the importance of working as part of a multidisciplinary team to ensure that high quality patient care is achieved .
I started working at LOROS hospice in 2023 and have expanded my knowledge in caring for patients who are living with life limiting illnesses . Patients admitted into LOROS range from requiring support with symptom management of pain, breathlessness, nausea/vomiting or may need emotional and psychological support. I have supported patients who have long term conditions such as; COPD, Heart failure and interstitial lung disease and have educated patients to adopt various techniques to help manage symptoms of breathlessness.
My knowledge of pain management has progressed and I have a good understanding of various medications used in palliative and end of life care
I currently attend weekly multidisciplinary team meetings at LOROS , where all health professionals meet to review patients that are staying on the ward . This has enabled me to acquire the confidence to communicate clearly and effectively within larger groups of professionals.
Overall I believe that I have many skills and qualities that I could bring to your team. I always strive to provide the best care for patients and am passionate about developing as a nurse by continuing my professional development. I would therefore like to be considered for this role of a practice nurse.
