I have worked as a Band 5 Community Staff Nurse at Wigan Central District Nurses for 2 years. Within that time I have progressed into two link nurse roles, attended all training including my lower limb assessor, to develop my role within the team.
I have applied to attend the V150 course to become a nurse prescriber to further my development.
I feel I am able to take charge of a weekend where no senior cover is available for team, confidently managing the caseload and triage and able to delegate where necessary.
I feel knowledgeable in clinical assessments, care planning and delivery. I am a compassionate, organised member of the team with great communication skills
~Managing my own caseload and work routine on a daily basis within my community role, managing 14 visits daily
~Currently End of life link nurse role, providing continuous holistic care to patients within last months/days of life, including supportive needs for patients family members Ensuring preferred place of care is met and working closely with the palliative community nurse specialists and hospice nurse specialists My role involves regular meetings with the End of life lead nurse and to cascade back to the team with updates/training needs
~I am currently Catheter Champion for Pemberton team, continuing to attend meetings with the Urology nurses and consultants I have attended clinics which has enhanced my skills for my community role
~I am competent in complex wound assessments and compression bandage training, I am a confident lower limb assessor and can evaluate/plan treatment for patient care
~I can confidently take lead of a weekend without senior cover, triage patients and schedule visits, I am also able to delegate the workload when appropriate
~I enjoy my time with students as I am able to support in development of pre grad nurses to promote clinical supervision
~I am aware of appropriate escalation processes and feel comfortable liaising with my team leads and higher management
~During my time on the community I have built lovely rapport with my patients which I continue to do so on a daily basis
~I have attended CHC and MDT meetings and confidently communicate needs and wishes for patients which ensures effective service delivery and an integrated care approach

Since starting my role as a district nurse within Wigan Central I feel I have flourished within my role in the community well, gaining specialised experience and knowledge.
I find working within the community both exciting and challenging and I am looking to challenge my role further as a community nurse to enhance my leadership skills.
I enjoy mentoring newly qualified members of the team and students to develop themselves.
I have applied for the V150 nurse prescribing course and I would love the oppurtunity to complete my Specialist Practitioner Qualification in the future.
I feel as though I am ready to take on a new challenge to progress in my career.
