Currently, I accurately dispense medications, perform clinical assessments, and conduct final checks of prescriptions in a high-volume community pharmacy, using advanced digital systems (such as Proscripts and PharmOutcomes) to proactively identify drug interactions and contraindications, ensuring safe and effective supply of medications. I am also instrumental in delivering Pharmacy First, hypertension case finding, and other advanced and essential services, consistently providing patient-centred care and making appropriate referrals when necessary. Over the past 8 months, I have established myself as the primary contact for nearly all Pharmacy First services and OTC medicine inquiries, demonstrating reliability and extensive knowledge.
In addition, I conduct comprehensive medication reviews and reconciliations at the GP surgery, supporting safe prescribing and deprescribing practices.
