• Monitoring vital signs and escalate when required
• Administration of prescribed medication and monitoring its effectiveness and adverse effect.
• Admission and discharge of patients.
• Catherter insertion, care and removal.
• Creating, reviewing and delivering personalized care plans
• Aseptic technique in wound dressing
• Maintaining personal hygiene of patients
• Maintaining comfort and safety of each patient through careful monitoring and knowledge application.
• Prioritizing and directing the workload of team members accordingly, ensuring any issues are dealt with as they arise and the health safety and welfare of patients and colleagues are responded to in line with agreed policies and procedure.
• Providing practical and emotional support to patients to help them maintain independent living.
• Assisting patients to carry out activities, hygiene routines and manage medication as directed within their care plans to help maintain health and wellbeing.
• Assisting patients to undertake practical tasks to help maintain their physical wellbeing.
• Undertaking shift lead responsibilities on behalf of the service for ordering, monitoring and administering medication to patients, ensuring prescribed drugs are used appropriately and safely in line with relevant policies and procedures.
• Assisting the management team by providing and recording information to help amend and update care plans so that changes in patient’s needs can be supported appropriately.

Responsibilities included;

• Assessing, planning, and implementing treatments with precision and care, prioritizing patient wellbeing.
• Carefully monitoring patient’s vitals with the aid of the cardiac monitor,
• Caring for central venous lines, peripheral and arterial lines,
• Feeding patients' via NG tube
• Maintaining the patient’s hygiene,
• Aseptic technique in wound dressing,
• Monitoring fluid intake and output of patient,
• Monitoring patients' level of consciousness using the GCS and RAAS Score,
• Suctioning of patients,
• Administration of medications and infusions through perfusors and infusion pumps and providing support and education to family as well as patients.
• Generating detailed and comprehensive patient care plans in collaboration with clinical teams.
• Educating patients on good health, providing leaflets and verbal guidance on improved wellbeing measures.
• Administrating prescribed oxygen and medication to achieve optimum health outcomes.
• Upholding patient’s trust and confidentiality by managing information with discretion, in accordance with data protection laws.
• Effectively prioritizing health problems, intervening appropriately to assist patients in complex, urgent or emergency situations.
• Quickly responding to medical emergencies, working calmly and professionally under pressure.
• Maintaining accurate, up to date patient records, maximizing data accuracy.
• Assisting with routine medical tests and evaluations, documenting findings to aid care planning.
• Mentoring student and junior nurses, building confidence and competence to ensure outstanding patient provisions.
• Building caring, compassionate relationships with patients, providing emotional support and encouragement during complex surgeries.
