• As Band 6 foundation pharmacist my duties and responsibilities were similar to my previous roles, however was given more responsibilities including;
• Leading cardiac rehab sessions where I help patients gain better understanding of their newly initiated medications. I gained my HEELaSE practice supervisor accreditation, which equipped me with skills to supervise and facilitate teaching sessions for MPharm 1-4th year students and pre-registration students. I was also responsible for organising rota for weekly teaching sessions for my peers. Carrying out monthly CD and medication safety audits on my designated wards.
• As part of my Medicines Information/ paediatric rotation, I was responsible for updating allocated drug monographs for MEDUSA which national resource used by NHS which provides guidance on preparation and administration of injectable medicines in adult and paediatric clinical areas. I liaised with pharmaceutical companies to ensure information provided was accurate and up-to-date.
• Cumulatively, these experiences have broadened my level of pharmacy practice and I have gained insight into understanding the skill sets of other professionals and observing the integral role pharmacist play within the multidisciplinary team as well as in education.

• As a Band 6 foundation Pharmacist, my duties and responsibilities included; reviewing prescriptions to ensure they are legal, safe, cost-effective, clinically necessary, appropriate, accurate, and complete. Arranging the supply of medications to patients, wards and departments.
• Providing information and advice to nursing staff and patients on a daily basis, about drugs, doses, administration and potential side-effects Provide similar information and advice to junior medical staff on a daily basis, and to Consultants as and when needed in both verbal and written form (e.g. Make entries in patient records).
• Accessing and interpreting clinical information for patients, and use it to individualise and monitor treatment.
• Whilst on my designated clinical ward I undertook risk management and ensured compliance with legislation and relevant national policies and guidelines, along with local policies and procedures including the Trust Formulary. And was responsible for deciding stock holdings with the senior nurse for the ward Dispensary.
• Working in a large multidisciplinary team, I had to clinically supervise, pharmacy technicians involved in Trust Medicines Management scheme on designated wards.
• For allocated sessions or agreed periods each week I acted as the supervising pharmacist or Responsible Pharmacist in the dispensary. This role involved clinically assessing in-patients, outpatient and discharge prescriptions by reviewing current in-patient medication chart against discharge prescription. Checking the prescriptions charts for: legality, legibility and intention of prescriber, that the medication is clinically suitable for the patient, any errors in dose, frequency, form and route, determine specific patient requirements and monitoring and resolving any queries with the prescriber.

Completed my pre-registration pharmacist training at Brighton and Sussex University Hospitals NHS Trust by undertaking a rotational competency-based training program to achieve underpinning knowledge and specified performance standards specified by the GPhC. Duties and responsibilities included assisting in the operation of pharmaceutical services and delivery of patient care, self-directed learning to support the collection of evidence to demonstrate competence and to undertake tasks to meet the performance standards. This was achieved through various rotations (namely renal wards, surgical ward, mental health , care of elderly ward, cardiology ward, attending anticoagulation clinics, medicines information, dispensary, community pharmacy, and audit project on the use of Teicoplanin in soft tissue infections).

Throughout my training year I was able to provide delivery of patient care and pharmaceutical services under supervision, whilst working within: my trust's procedures and policies and the accepted standards of practice, relevant legislation and regulatory requirements.
