  • Preparing the paperwork required for the nurses completing the assessments.
  • Reporting to senior management any breaches that may occur and the reasons why.
  • Keeping Broadcare up to date with all information concerning the MDT booking and any conversations with the families and health professionals involved in the patients care.
  • Building trackers to be used as a reporting and tracking tool for the progress of each patient.
  • Uploading all paperwork completed by the nurse assessor and any reports or clinical evidence supplied by the health professionals involved.
  • Answering general enquiries from the hospital wards on how to complete the paperwork and the estimated timeline for the patients MDT and Panel outcome.
  • Ensuring a close professional relationship with the local authority and the hospitals at all times.
