My role as a support worker was to help assist and support patients with their rehabilitation journey so that they can become well enough to be sent back into the community safely. I helped to assist the patients with their personal care needs, oversee patients' daily activities, introduced patients to new mobilising equipment and explained this to them, documented patients' progress in reports, discussing the effectiveness of certain treatments with higher management and other medical professionals. I worked as part of a team and we worked together ensuring our patients are receiving the best care they deserve. I was part of their journey to helping rehabilitate them to get back out into the community safely. I had excellent observation skills as some of our patients come with mental health conditions as well, which sometimes lead to some of them being on different observations levels for example 1 to 1 supervision at all times. I read their care plans to ensure that we were delivering the care that was tailored to them. I worked with fluid and diet charts. Another duty of mine was to carry out physical observations on our patients and to also carry out the admissions of new arrivals on the ward. I worked very closely with other medical professionals and we communicate and work together as a team to ensure that we were delivering the utmost excellent care for each and every individual patient on the ward. I made sure to keep up to date with relevant training and followed all wards policies and procedures and follow the trust values and the 6 c's in care every day whilst at work. I always used respect and dignity with every single patient and I always supported and encouraged independence. I also maintained confidential information and record and document this down in secure areas, promote the anti-discriminatory practice, helped to promote and support each individual's choices and rights, helped individuals to communicate their individual needs.

I worked on the PICU ( psychiatric intensive care unit) ward so therefore my role was very demanding. I had to ensure the safety of our service users and staff was paramount. I was there to support our service users throughout their daily lives. Following the 6 c's in care in every single day at work to ensure I was providing excellent care for our service users. I had to make sure that I upheld my duty of care supporting our service users and their individual needs every day. I made sure to keep up to date with all relevant care plans, training and any extra noted information which I would find one systm one to keep me in the loop with all communication on our ward. I also made sure to help our service users in assisting to their personal needs with basic hygiene activity if needed. I had to be observant at all times on the ward ensuring the safety of everyone and to also keep an eye on our service users and how they were presenting. I worked closely with other health care professionals to ensure we were providing the utmost excellent care and support that our service users deserve. I treat everyone with dignity and respect whilst at work and followed our company's trust policies and values at all times. I also maintained confidential information, promoted anti discriminatory practice, helped to promote and support each individuals' choices and rights, help individuals to communicate their individual needs.
