Summary

I am an enthusiastic, motivated GPST3 looking for a salaried role for 2-4 sessions following completion of training. I enjoy person centered consulting, the cognitive challenge of working as a generalist, and working as an effective member of the practice team.

I would be interested in further developing my interests in safeguarding, child health surveillance, substance misuse and patient safety/ quality improvement.

Whilst a registrar, I have gained experience working in a general practice role, and I am confident reviewing patients during clinical sessions (face to face, telephone, home visits, duty doctor, out of hours work); administration (prescription requests, document management, results management); quality improvement (QOF, quality improvement activities, organising a small population health management project); teamwork (liasing with colleagues within practice and PCN, attending practice meetings). I have experience with addressing safeguarding concerns via local pathways; and I have used opportunites to understand local allied / 3rd party services available to patients within the local area.

Quality improvement

Population Health Management programme - Funding from ICB was acquired to focus on frail, elderly patients who had not recently been seen by a doctor. The focus was to optimise health in this cohort, by detecting undiagnosed conditions, code fraility score, health promotion, consider advanced care planning and refer to relevant PCN services. I liaised with admin team, organised face to face appointments and home visits and fed outcome of the project back to practice. 

Introduction of formal reviews of all deaths in surgery- Following discussion in practice meeting, I edited the SEA template to include a 'death review' template and complete these for each patient who dies, with any learning points. I then present and discuss these cases in the meeting with colleagues. 

QOF - I am aware of QOF framework and have completed work on my allocation- reviewing notes, contacting patients if needed and coding the outcome in the notes. 

GP VTS QIP- I completed an educational QIP, liasing with a relevant charity and GP trainees to improve GP trainees knowledge of intrahepatic cholestasis of pregnancy. I presented the QIP in poster form at RCGP annual conference 2021.
