Resident doctor / Inpatient Care Lead (Acute Psychiatric Unit)

Responsible for the clinical management and safety of patients in an acute inpatient psychiatric unit, including those with severe mental illness, and long-standing substance use disorders. Conduct comprehensive admissions, including medical, psychiatric, and addiction histories, mental state examinations (MSE), and withdrawal risk assessments.

Manage acute presentations alongside detoxification and stabilisation from substances such as alcohol, opioids, and benzodiazepines, using evidence-based protocols. Perform ongoing risk assessments and implement safety plans in collaboration with the multidisciplinary team (MDT) and Community Mental Health Team (CMHT).

Lead ward rounds, coordinate patient care, and oversee medication management, ensuring safe prescribing, and legal compliance. Plan and facilitate safe discharges with a focus on relapse prevention and continuity of care through community mental health and addiction services. Maintain accurate documentation and effective communication with patients, families, and healthcare professionals.

• I observed the clinical work being conducted in the hospital, including patient care from early admissions, history taking, examination, and discharge processes. I gained an understanding of both online and paper-based documentation and workflow.
• I observed the communication between patients, junior doctors, and consultants regarding medical conditions, diagnoses, and management plans. Additionally, I noted the work done in the acute care unit, which involved managing day care patients for acute conditions and consulting speciality departments as needed.

This period of experience provided me with an opportunity to work in the primary healthcare department, where outpatient services for day-to-day cases, basic obstetrical care including antenatal care, and preventive health services were performed. The role of the Medical Officer was to oversee all these activities, and ensure they ran smoothly and efficiently.
