Community Palliative Care Support Worker

-Supported children and young adults (0-19yrs) with terminal illness, life limiting conditions and disabilities across Luton, Bedford and Milton Keynes.

-Direct contact with families, helping to coordinate care, offering emotional and practical support.

-Participated in various meetings such as symptom management and safeguarding with a multi-disciplinary approach.

-Served as an advocate for families, a link between professionals.

-Arranged and supported stays for patients requiring symptom management, crisis support, respite or end of life care.

-Helped to run a Children's Centre supporting the local community in Ivinghoe and Pitstone.
