Responsible for the unsupervised assessment of patient care needs and the development, implementation and evaluation of plans of care, ensuring high standards of evidence-based care using a defined model of care

• Deliver quality, efficient and effective person centred care which meets the needs of the patients and their families or carers.
• Assisting with acute treatment for all types of stroke: ischaemic, haemorrhagic, and transient ischaemic attacks.
• Act as an advocate to assist patients and their families/caregivers in making informed decisions.
• Administration of prescribed medications through different routes (oral, IV, NG), and monitoring the effectiveness of medications.
• Support patients and their families /carers to ensure clinical interventions and service delivery process are understood and enable patients to make informed decisions in relation to their health needs.
• Assess for neurologic dysfunction, such as level of consciousness, motor and sensory deficits and vital signs
• Make appropriate referrals to support patients requirements on discahrge.
• Provide appropriate supervision for staff as required, including the supervision of students on clinical placement.
• Respond effectively to emergency situations and the changng status of the patient .
• Delegate aspects of care to the team members according to their competence and within their scope of practice.
• Delivery of service -within professional boundaries, ethics and cultural respect,scope of practice.
• Carrying out nursing procedures such as IV Cannulation, Urinary catheterisation, Nasogastric tube insertion and suction according to the needs of the patients
• Assessing patient's swallowing and speech problems and collaborating with multidisciplinary teams for further management
• Supporting and encouraging rehabilitation and monitor the progress of individuals in rehabilitation programs and adjust care plans as necessary
• Evaluate the effectiveness of interventions and make necessary adjustments
• Ensuring falls prevention protocol for those who are at the risk of fall
• Maintain accurate and thorough medical records, including assessments, care plans and progress notes
• Assessing patient's skin and management for skin concerns such as pressure sores, skin tears and diabetic wounds
• Provide emotional support and counselling to individuals with disabilities and their families, addressing psychological and emotional aspects of their health

• Conduct comprehensive assessment of individuals with disabilities to identify their physical, emotional and social needs
• Collaborate with patients, families and healthcare professionals to develop personalized care plans
• Work with a multidisciplinary team, including physical therapists, occupational therapists, and social workers to ensure coordinated care
• Facilitate effective communication among healthcare providers, caregivers and individuals with disabilities
• Provide nursing care focused on maximising functional abilities and promoting independence during rehabilitation
• Assist in creating and implementing rehabilitation programs tailored to individual needs
• Administer prescribed medications, monitor their effects, and educate patients on proper medication management and potential side effects
• Assisting patients in their move toward independence by setting realistic goals and treatment plans
• Advocate for individuals with disabilities to secure appropriate services and support
• Encouraging patients to reach their optimum level of functioning to restore personal autonomy, whilst providing education, information and support to both patients and their families
• Collaborate with community resources and organizations to enhance the overall wellbeing of individuals with disabilities
• Performing team leader roles and supervising the junior staff members and assisting the staff members in providing proper guidance and support
• Evaluate the need for and facilitate the use of assistive devices and technologies to improve daily living activities

• Provide specialized care to critically ill cardiac patients, managing high acuity cases such as Myocardial Infarction, Pulmonary edema, Angina, Cardiac arrhythmias and assisting in life-saving interventions
• Administer cardiac medications, administered oxygen therapy, and monitored patients' vital signs resulting in early detection of potential emergencies
• Collaborate with multidisciplinary teams to implement comprehensive care plans and received commendation for reducing readmission rates by 20%
• Educate patients and their families on post-operative care measures, resulting in improved patient satisfaction scores and faster recovery times
