Supporting Information

  • My current role as a mental health case manager is relatively new to me but I incorporate my skills learnt through working in offender mental health as detailed below.
  • I integrate the stepped care model into my practice to provide care to patients with complex and enduring mental health and substance misuse needs within the offender health directorate, this also incorporates collaborated working with the recovery service for individuals with dual diagnosis.
  • I have received a certificate of recognition for 100% attendance.
  • I have developed effective risk assessment skills, critical thinking skills and decision-making skills through self-awareness and confidence within my role which often includes dealing with the frequent challenging scenarios and complex individuals the prison environment can present with.
  • I have a good knowledge base of the relevant agencies that are worked with collaboratively within the criminal justice service and wider community including probation, offender management, attending parole hearings, sentence planning meetings to ensure prisoners meet their targets to engage with mental health, this is to promote risk reduction and progression through the criminal justice service.
  • The mental health team is also involved in first line assessments through the reception screening process in collaboration with the primary care team when prisoners are first admitted into the establishment. This is a comprehensive risk assessment to identify any current mental or physical health needs and if they require input from the service.
  • This has provided me with good interpersonal skills and communication skills, both written and in oral communication should a difficult situation arise where a prisoner is emotional or distressed.
  • I feel as though I have the ability to adapt to challenging and changing circumstances that can occur through managing incidents and making clinical decisions: new referrals can often present as emotionally distressed and in crisis and often voice thoughts of self-harm and suicidal intent.
  • This work has also provided me with the skills to use my own initiative and to work effectively as part of the multi-disciplinary team leading the primary care section of the mental health team.
  • I do this through undertaking the planning and management of the triage referral system and the management of junior staff on duty by taking responsibility for the appropriate delegation of tasks and ensuring they are competent to deliver the required care in the required time frame.
  • I maintain a case load which is currently divided into primary and secondary care cases and provide interventions through care co-ordination through the CPA process to promote social inclusion and ongoing support and recovery.
  • The case load consists of individuals with common and complex mental health issues, and I prioritize the workload based on flexible principles and prioritizing tasks that ensure an effective and successful outcome.
  • I attend meetings as a mental health representative to input into MAPPA and the establishments good order and discipline system and the ACCT (assessment, care, custody, treatment) process.
  • I also managed 2 members of staff providing clinical supervision incorporating Gibbs reflective cycle to promote reflection on practice and participate in the individual’s performance and development reviews in accordance with local policy.
  • This is generally done monthly or when the supervisor/supervisee identifies the need for further input regarding a complex case or work issue.
  • I ensure that all records are maintained and stored safely within clinical areas to promote information governance.
  • I have previous experience of working with offenders in primary and secondary care through my positions held at Newton Lodge RSU and Hmp Leeds.
  • The Primary care team provides the starting point for assessing the mental health needs of individual prisoners which includes planning and coordinating packages of care and treatment that best meet the needs of the individual.
  • I have been involved in a wide range of interventions to support the treatment of mental ill health and learning disability and the promotion of positive mental wellbeing.
  • This includes conducting comprehensive risk assessments using local and national assessment tools to ensure patient safety and to ensure that they receive the appropriate interventions.
  • I also incorporate my training in CBT into 1-1 sessions with individuals and use the GAD 7 and PHQ 9 as a benchmark tool for measuring recovery outcomes.
  • I have completed the non-medical prescribing course at Leeds Beckett University and prescribed for patients who attend triage clinics with depression and anxiety.
  • Other departments can often become involved with the care of individuals to ensure the best care package is delivered, this can include psychology, probation, the offender management department, and the in patient’s department, this then supports the prioritizing of the patients and their care needs.
  • I have found that providing early intervention and support through primary care can assist the individual to continue to engage with the process of care and improve progression.
  • I maintain close links with the prison wing senior officers and custodial managers to ensure a collaborative continuity of care whilst maintaining confidentiality.
  • Through training and ongoing education and clinical supervision I have identified an increase in the effectiveness of clinical work and risk assessment in practice.
  • Whilst based at HMP Leeds I was involved in the implementation of a mental health promotion drop-in clinic.
  • I presented a power point presentation on a CBT case study to highlight its effectiveness within primary care as it is evidence based and proven to work with the offender population.
  • It is a structured approach to treatment and maintains professional boundaries which aims to keep the individual engaged.
  • It can also manage and highlight the level of risk that can occur within the offender health population.
  • I was able to implement my transferable skills through training in CBT in collaboration with 2 student nurses I mentored to offer guided self-help information on common mental health problems such as depression and anxiety.
  • The service was provided with excellent feedback through the prisoners completing questionnaires and feedback surveys for the team who were then able to provide a more structured and robust service based on individual need in collaboration with the prisoner.
  • Patient involvement is a key element to improving their overall experience, and improving the way practitioners work creates a modernization movement that is connected with people’s values and beliefs which is what I attempted to do through the drop-in clinic.
  • I am an enthusiastic and knowledgeable RMN with direct experience of working with adults and older people experiencing various mental health conditions and brain injury.
  • I have a sound knowledge base and experience of working in secure environments and understand the safety and security issues that can arise within different health settings.
  • I am sensitive to individuals’ needs, always maintaining confidentiality.
  • I feel as though I am a good team member who encourages others to contribute to the ongoing improvement of the service by demonstrating good decision-making skills and confidence in my role.
  • I am realistic about my own strengths and weaknesses, and I can use my own judgement to refer to higher levels of leadership where appropriate.
