Summary

I have been working within the community setting for the NHS, Visiting patients within their own homes, for the last 7 years, progressing to an OTA within the last 4 years, within this time, I have built up my skills and knowledge within the field of Occupational therapy and trive working in the comunity to enable patients to continue living in their own homes safely.

My experience includes visiting patients in their own homes, completing therapy assessments and ordering equipment that meets individual needs, reviewing once in place to educate new ways of using whilst maintaining safety, not only to patients but their families and caregivers too, promoting independence and self-care. Throughout assessment OT theory is used when assessing patients looking at the person, occupation and their environment, to gain an understanding of the patient’s wants and needs, then together setting realistic goals to work towards that are meaningful to them, for instance, supporting patients with daily living tasks such as personal care, meal prep and accessing the community again, and at times adapting their environment with equipment and measuring up to instal fixed adaptions, these interventions are tailored to individuals goals to help them build upon their confidence, and maintain function, whilst promoting independence within their chosen occupation.

I manage my own case load and adhere to policy and procedures; I have IT knowledge of system1 and TECES and use these daily. My communication skills, bothwritten and verbally are clear and professional, ensuring the patient, their families and carers feel supported throughout therapy input. I work well within a multidisciplinary team and with other services that are involved within the patients care, to enable the patients to live well and safely at home.

Further more I am a dedicated and compassionate individual with a commitment to supporting others in improving their independence and wellbeing; seeking to apply these qualities as an occupational therapist. In previous roles I have been faced with demanding environments which have improved my problem-solving, communication and collaborative skills. I take pride in working to high professional standards, and I am eager to transfer these skills into the occupational therapist role. I am committed to continuing to build my knowledge and skills of this practice whilst in the healthcare setting.

  • Supporting Rehabilitation assistants, O.Ts, physios and nurses with patients.
  • Liaise with patients, families and other services to ensure patients are supported and cared for, whilst proactively listening to any concerns, needs and worries.
  • Carry out home visits and complete patient assessments, where I can apply critical thinking and identify any OT needs and therapy interventions needed. working closely with the patient and families to enable person-centred care.
  • I have IT knowledge of System 1 and TECES, and liaise with patients, families, caregivers, and care providers over the phone.
  • Order equipment through TECES that is tailored to the patient's needs and therapy goals.
  • Reassess once equipment is installed, educate on new ways and safe ways of using equipment, tweaking if needed. to promote independence and maintain ability.
  • Monitor patients in their own home through BPs and sats whilst liaising with nursing staff on any changes to condition, skin, and ability.
  • Continuing knowledge by keeping up to date with mandatory training, and working with other OT's to learn new skills will improve my efficiency and productivity

  • I have recently completed a degree in Occupational therapy over the last 4 years whilst working as an Occupational therapy assistant within IRS / RAFT, where I completed an apprenticeship. I am currently awaiting my PIN.
  • Knowledge of Occupational therapy frameworks and applying therapy models such as person- person-environment-occupation, to support holistic, person-centred care by focusing on the interactions of the person, their environment and their meaningful occupation. To enable assessments, goal settings, and applying interventions that will enhance independence, dignity, and promote their ability, whilst enhancing overall quality of life
  • Completed work experience through placements in different settings. First placement was an inpatient rehabilitation ward, where I would gain information from families with home heights to work with on the ward, so patients before discharge could work on safe transfers with their heights before returning home. completed washing and dressing assessments, kitchen and stairs ability and conducted therapy exercise sessions. Completed discharge paperwork, equipment orders, liaised with care companies for packages of care, provided referrals to continence teams, therapy services and District Nurse support, and provided information on key safes and care alarms.
  • Second was CAMHS Mental Health Children's Disability Unit, completing sensory assessments, school and home visits. working closely with social workers, nurses and psychiatrists.
  • Third was ICT Billaricay, where I worked closely with other OTs, physios and RA'S. Lone working following policy and procedures, had my own caseload, and booked patients in and completed home assessments, liaised with patients, families and caregivers, identified needs and measured up for equipment, provided person-centred care and patient choice. I reviewed the equipment was in safe and showed how to use it safely through educating new ways of safely doing a task, and the patient was happy. I used IT systems through system 1 for patient notes, completing promptly and as per policy, I used TECES for ordering, chasing and requesting collections, and repairs.
  • During my placements and apprenticeship, I developed a range of clinical and interpersonal skills, whilst building up my own confidence and the competencies required to become a qualified Occupational Therapist

I worked as part of the Bridging Team in the community following discharge from the hospital. My role included;

  • Assessing patients on the ward prior discharge to gain information on their ability to transfer, mobilise, and check skin, whilst aslo collecting next of kin information and home address details.
  • Ensuring the patient returned home safely.
  • Moving and handling were appropriate for the patient once returning home.
  • All equipment needed was in place.
  • Medication management ensuring the patient had all medications as per the discharge letter.

  • Moving and handling, use of hoists, returns, sara steadys, frames and walking sticks
  • Ensuring the home is safe to manoeuvre, removing, and reducing the risk of slips, trips and falls.

  • Calling for medical attention through 999 or 111, Thurrock first, and UCRT and assisting with unwitness falls, cardiac arrests and patients who had passed away.
