Supported young people and adults in the many different aspects of daily life within our homes including

-Assisting and encouraging people to engage in everyday home, social and leisure activities

-Follow the homes policy and procedure, care planning and risk management for each young person.

-Provided care , nurture and establish positive relationships with young people and adults assessing needs and working collaboration in producing , implementing ,delivering and revising individual care plans.

-Maintained a high quality of care and support which meets physical , emotional, intellectual , social and cultural needs of young people within the care of aspirations.

Promote health and patient education for people with learning disabilities and autism

Key Responsibilities

- Working with child and adolescent mental health services ,the police and social services to young people safe.

-Uploading and organizing service user's care files as well as prompting and administering medication

-Providing companionship and supportive listening to service users by showing empathy and building rapport in communication.

-Encouraging residents to mobilize safely around the home, with the support of mobility aids and specialist equipment .

Delivered high standard of holistic care to patients to promote equality and dignity.

Key Responsibilities

-Liaised with other appropriate health professionals at times of service users illness or/at the end of life.

-Assisted with monitoring and reviewing service users nutritional needs in line with the care plan

-Answered emergency bells, the door and telephone, and greet visitors in a professional manner.

-Ensured that service users physical environment is maintained to a high standard

Outline

Offered support and advice for people who have experienced domestic abuse and sexual violence in West Yorkshire

Key Responsibilities

-Work within a feminist framework to readdress the gender and structural inequalities that limits the lives of women identifying people.

-Used a person centered, survivor focused approach bearing mindful of the effect of trauma and additional barriers to survivors.

-Worked in partnership with health professionals, the police and therapist to make ensure that all the necessary processes are carried out to maintain safety of victims.

-Ensure effective access to service users and encourage their engagement with service
