After a short time working as a remote monitoring nurse, I was promoted to my senior position. I continue my work in remote monitoring alongside further responsibilities which include;

• Shift leading. Organising staffing allocations for the shift and being the first point of contact for any escalations.
• Contributing to staff training. Creating remote teaching materials for our current, and ever growing, clinical team.
• Manage a small team of clinicians, providing support and feedback for work satisfaction purposes and improved performance.
• Working with the other senior nurses to streamline clinical processes and optimise workflow.
• Organising and attending monthly meetings with the tech team to offer feedback on our systems for improvement purposes. During these sessions I also arrange clinical shadowing sessions, which allow the tech team to see how we use the system as clinicians, aiding development of the product.
• During the development of a 'passive monitoring' dashboard, I was engaged with the development team to offer my expertise in continuous monitoring and dashboard layout.

• Monitoring patients remotely, observing for any early signs of clinical deterioration.
• Using clinical assessment skills via telephone to gain vital information.
• Escalating concerns appropriately using guidance set out by the NHS trusts.
• Using time management skills to prioritise patient care, ensuring that patients are reviewed in order depending on their needs.
• Establish positive rapports with patients and families through active listening and compassionate care. Offering reassurance and clinical guidance to support them in their recovery or maintenance.
• Working closely with fellow nurses, doctors and clinicians for consistent communication and continued best care practices.
• Efficient communication with colleagues to ensure that my team are aware of any clinical concerns or problems that may arise.

• Navigating multiple software systems, ensuring that the communication between the company and the trusts we monitor for is seamless.

• Managed a team of over 70 staff.
• First point of contact for patient escalations from nursing and care staff. Used effective triage and clinical judgement to decide on the best course of action for treatment.
• Supported nursing and care staff to deliver excellent care to residents, through present and approachable leadership and mentoring.
• Reviewed staff performance and implemented training to enhance strengths and reduce weaknesses.
• Created and managed an efficient auditing schedule, ensuring that all residents had up to date care plans and risk assessments - and ensuring staff training was relevant and well documented.
• Worked collaboratively with the management and wider teams to ensure compliance with Care Inspectorate regulations.
• Introduced innovative AI technology (PainChek) to assess pain levels in patients with cognitive impairment after conducting thorough research. Provided training sessions to staff on the importance of detecting and managing pain and using the tool appropriately.
• Liaised with external services, such as social work and GP's, to provide holistic, person centred care.
• Led multidisciplinary teams in delivering exceptional care throughout patient visits.
• Resolved complaints quickly, calmly and professionally for positive resident and business outcomes. Reporting to the appropriate clinical bodies where necessary.

• Provide holistic care for patients with complex medical needs in the community, including those who require at home ventilation.
• Support and mentor care staff to meet patients needs. Provide training on enteral feeding, tracheostomy care, ventilation management and catheter care.
• Assess patients' physical, psychological and social issues to identify appropriate nursing interventions.
• Inform patients and relatives of treatment plans and expected outcomes.
• Using a wealth of clinical assessment skills, determine the best route of treatment for patients who present as being unwell at home. Referring on to the appropriate health care professionals when required.

• Counsel caregivers and families on end-of-life planning and patient care options.
• Facilitate adequate pain management for assigned patients to minimise discomfort.
• Guide patients, caregivers and family members on plans of treatment and helpful resources available locally to support care.

• Train and coach new employees on protocols and best practices to promote ideal patient outcomes.
• Participate in various departmental quality improvement initiatives to achieve a higher standard of patient care.
