I carry out assessments in people's homes under the
Care Act 2014 to reduce deterioration in health in health and avoid ending up in hospital. For example, assessing a a client who is struggling with bath transfers and she also is struggling with attending to personal care and administering medication.

I would have to carry out the Social Care Assessment, even though they now have equipment in place, they might need a carer to support them with assessing the bath and attend to personal care.
I might also have to refer clients to HomeMove if their property is no longer suitable for their needs and I have to complete a form and email it to the HomeMove.
I also encounter clients who, English is not their first language and I have to arrange the interpreter first before the first visit.
After all the assessments has been done, I need to review them to ensure that all is working well.

My role was to work with clients who have been discharged from hospital or those in the community with a potential goal to be to be independent within the period of six weeks.
I completed the first visit and work out how we can support the client to achieve those goals, for example, a client who had a hip replacement, I had to set goals for every week, goals that were achievable to enable the client to be independent. If they're struggling with medication, the first week I would administer, the second week, I would encourage them to identify their medication and how many times they take them. Any problems, I would report it to the senior carer.
I ensured ensured that they have proper nutrition by preparing healthy meals in the first week.

My role was to work with adults up to 6weeks.
I completed the first visit to ensure they have medication and they have food in their home.
Goals were set for every week to ensure that by 6weeks, the client will be independent with most tasks - for an example a client who struggled with bed transfers, I would ensure that she is doing her exercises and walking around in her home.
I would also ensure that by six weeks, she will be independent with tasks.
Then the last week I expect the client to be independent with tasks.
If the client is still struggling, I would inform the Senior Support worker who will make a referral to the Occupational Team or the Physiotherapist team.
Exercises are very important to to enable the client to achieve their goals.
