I have been part of CRRT since it commenced 1st April 2022.
As an Advanced Practitioner I am lead within the team for practice development ensuring good practice is promoted within the team.
I provide management oversight and practice direction to the team.
I take the lead for identifying learning and development needs for the team, modelling good practice, which is highlighted within the audit process.
As a Advanced Practitioner a provide supervision to a newly qualified/ASYE social worker. I also supervision a Community Care Officer. As a Practice Educator I have also provided supervision and mentoring to student/apprentice social workers. I provide case load and reflective supervision in addition to the 1:1 supervision.
I undertake intake duty, for CRRT which is very busy. Prioritising referrals based on risks, then triage and allocate them appropriately in accordance with CRRT referral criteria. Providing oversight to members of the team. Also to ensure that the referral is actioned within 2 hours of receipt of the referral.
I participate in regular MDT meetings with stakeholder and partners. Explaining the role of Adult Care Services and providing guidance on cases.
I have built excellent relationships with stakeholders and our partners as I have been part of the CRRT since it commenced 1st April 2022.
I have taken the lead role ensuring that CRRT have a strong affiliation with health colleagues within HCT and hospital at home.
I have supported new members of CRRT and EIV with induction.
I am allocated complex cases which involve safeguarding, Deputyship, Lasting Power of Attorney. I also have complex mental health which require close MDT working.
I provide management cover when the team member is undertaking her project duties.
I ensure that all training is up to date for the team members.
I attend AP monthly forums and disseminate practice information/notes from practice development and the principle social worker to the team.
I have excellent communication skills; I am committed to lead and support the team to focus on ensuring that each person that is referred to CRRT is supported by the team to avoid admission to hospital. This is if it is their wish and they do not need medical interventions.
During the period of October 2022 to November 2023, CRRT have not had a Deputy Team Manager. Therefore, I have stepped up to cover many of the managerial roles.
