- I am a Pharmacy Technician who works to provide the highest standard of patient centered care using a team approach.
- I work as part of a multi-disciplinary team ensuring medication is provided to the patient, accurately and safely, and working to the most cost-effective prescribing.
- I support clinicians as a point of contact for medicine related matter, including sourcing of medicines and communicating with pharmacies.
- I will oversee and liase with the prescribing team in processing repeat requests.
- This also involves referrals to Medicine Support and helping with assessments for patient compliance to improve.
- This means regular training with colleagues and updating them on all points related to medicines.
- I undertake medicines reconciliations post discharge or clinical contact.
- I monitor practice adherence and help to support clinical governance, I action MHRA safety alerts and communicate to clinicians.

- Ensuring monthly medications are prescribed appropriately for care homes, liaising with them regarding new medications or changes to doses.
- Arranging assessment of patients needing covert medications to be considered or reviewed.
- Ensuring Homely Medications are up to date and working with HCPs within MDTs to provide safe and continued care.
- Working alongside the PPG to improve the relationship between patient and practice enabling clear communication and dealing with patient complaints.
- Supporting Patient Services Manager in reception staffing and training.
- Supporting the senior practice managers with the day to day operational needs when required.
- Attending Palliative and Integrated Care meetings to support a whole practice involvement of vulnerable patients with Care Co-Ordinator.

- When a new practice manager joined us it was decided I was to be promoted to manager.
- I worked alongside a pharmacy dispenser who I had recruited and between us we covered the 50 hours per week our contract held.
