The field of Emergency Medecine is a diverse and complex speciality. Unlike most other specialities the clinician is expected to be proficient in nearly all if not every area of practice. It is very difficult to encompass all the necessary requirements however the more experience and exposure the person has the better the clinician is equipped to deal with the unexpected.
In my case I went to the UK to complete my internship and to experience a different clinical system. I did enjoy it however the lure of home was strong. When I returned I joined the surgical scheme in the South East. I enjoyed the BST in surgery however it was my placement in the ED that really appealed to me and I "found my calling".
This Clinical Fellow position is in the department of Anesthesia. My main job plan is focused in theatre and management of patients both pre and peri-operatively. The caseload varies from General surgery, Orthopedics , ENT to Gynecology. This post enables me to learn more airway skills as well as procedural based techniques which include arterial lines and central access.
My current post is a Clinical Fellowship. I still do Registrar duties however it’s a tertiary Centre encompassing Spinal, Orthopedics, Neurology, Nerosurgery and Dermatology among other specialists. It is also a large scale Trauma Centre with on call MRI 24/7.
I started this post after taking some time out. It is a Senior Registrar role and encompasses a more supervisory role as well as dealing with patients. I am involved in academic tutorials, teaching, patient care and protocol development as well as my normal medical duties and obligations.
This was the last of my three six month postings in ED. It is often envisioned as the finishing part of the scheme where all the knowledge and experienced should come together to derive the desired outcome.
This six months provided me with exposure to both neonate and general medical paediatric cases. It involved clinics, on-call and ward based duties. It also included consult and maternity cases.
This was the start of my second year and I returned back to ED. I was delighted to return to my favourite job. I once again was exposed to a variety of different clinical presentations. This post also enabled further exposure to the use of ultrasound in a clinical setting.
This six month post involved patient care on the orthopedic wards as well as attending OPD and theatre. I also did call and made clinical decisions with regard to admission and discharge. I enjoyed it and aquired as much practical and clinical knowlegde as possible.
I was working in this post for over four and a half years. I deeply enjoyed the role and decided to persue a career in the field of Emergency Medicine. The post itself changed some what over the preceeding eighteen months with the arrival on the new ED however the fundamentals of patient care is still paramount. My role involves shift work. The patient cohort seen is diverse and ranges from medical, surgical, orthopedic to paediatic patients.
Initially on completion of my surgical BST I wished to change direction and that is what led me to ED. I also completed my MSc in pre hospital and Emergency in December last year.
This six months completed my two year basic surgical training scheme. I worked as a surgical SHO and was involved in ward duties, on call rosters, theater and out patient clinics. During this scheme I ascertained the basic clinical surgical skills as well as furthered my clinical experience and exposure.
This six months formed the part of my second year of BST. Once again it encompassed out patient, ward management, elective and emergency theater work as well as on call duty.
This six months provided my first exposure to the world of emergency medicine and probably changed my outlook and forever. I really enjoyed it although did find it initially daunting and challenging. The diverse nature of the job did appeal greatly to me.
This six month posting involved three months trauma followed by three months of elective orthopaedics. Although there was on call duties the predominate component was around elective work and out patient clinics.
This was the last of my two year foundation scheme in Scotland and was centered on the speciality of Obs and Gynae. It predominately involved elective theater, out patient clinics and on call responsibilities.
This four months provided an interesting change as most of the schemes in the UK system involve some exposure to GP in order to fully comprehend what practice outside a hospital is actually like.
This was the last four month job of my first year within the NHS and was involved in ward based management of general medical patients. It encompassed call, ward work and AMAU patient cohorts.
This six months exposed me to the more unusual conditions which can present to a hospital. These include malaria, typhoid and rare forms of TB. My on call duties also involved cross cover of the respiratory wards at night and weekends.
This was my first job on arrival to the NHS. It involved on call, theater, OPD clinics and ward work. It also opened my eyes to a large scale hospital and what is within its walls.
This was the beginning of my clinical practice and was very enjoyable.
