This role required me to undertake Care and Support Plan reviews and assessments in line with The Care Act 2014 to promote the wellbeing and support of Adults who have a diagnosed Learning Disability and physical impairments.
Part of the role required me to cover our Client Support Service, triaging daily contact from people we support, their carers and external professionals. At times I was required when requested to undertake section 42 safeguarding enquiries working alongside a multidisciplinary team which may include our health colleagues, the police and CQC to ensure the person at the centre of the safeguarding concern is safe and their rights upheld.
I was fortunate within my role to work closely with our health colleagues including Occupational Therapy, Nursing and Physiotherapy. Having the experience of joint working with these professions has enabled me to widen my skills and knowledge in order for me to apply a more holistic approach to assessing the needs of the people we supported, which may include their social, environmental and physical needs in order to support an achievable outcome to enable them to meet their particular need at that time this includes recognising when a referral is required to each profession and the purpose of that referral whether the be the need of equipment or support with a changing health needs.
My role required me to visit the people we supported working in a flexible manner that enabled me to meet with the person at the place that is more comfortable for them whether this be home, day service or respite.
My duties required me to monitor and discuss with clients, their representatives and social care teams regarding outstanding monies owed in respect of assessed client contributions towards a person's care. During this process it would be my responsibility to raise concerns with the social care teams whereby financial mismanagement was believed to be taking place which may result in safeguarding enquiries being raised.
My role was to meet the care and support needs of elderly adults within a residential service. To support and enable them to meet their needs as noted in there support plans, whilst following guidance a recommendations given by external professionals and adequate safeguards in place to minimise the risk of harm and decline in health and wellbeing. I was responsible for accurate record keeping and recognising potential change in need raising with within the senior team to ensure the needs of that person were met.
