Summary

I currently work as a senior service finder within the transforming care team. Within my role I search and commission services that are appropriate to the person, and not an under or over provision.

I have developed skills to work outside of frameworks if this needs to happen.

I have completed reviews as appropriate as people have different needs at different times, so reviewing and staying up to date with my cases is very important to me.

I have accessed different funding streams to acquire services and equipment, these efficiencies support the social care/health budgets.

Within my role I have a good understanding of costs/budgets and rates of support I always ensure these are implanted correctly on individual support plans and reviewed regularly.

I work closely with community accommodations teams with ensuring I am updated with provider rates and costs, ensuring when I commission a service the correct costs are reflected on a persons assed needs.

I have also initiated and organised network meetings on a regular basis with professionals from various agencies often taking the lead in these meetings to ensure all stakeholders work together by devising a suitable action plan and delegating where necessary so that a smooth transition on discharge is experienced for the person by working in close partnership with the Health and Social Care collegues.

It is important to me to have good partnership working as this will help with positive results and achieve best outcomes for the people we are supporting.

Most people I work with have different cultural backgrounds, such as family status, I always ensure the person is the focus of the assessment and that they can express their likes and dislikes and their goals in life. I have worked with families who have cultural needs. I ensure that the person’s voice is heard, on some occasions an advocate/interpreter would benefit from being involved as they would represent the person voice of the person is important as they are my main focus.

It is important the person I am supporting is being listened I have adapted my communication by using tools such as picture pegs to enable better communication.

I also work alongside health commissioners; I understand that the teams have pressures from senior managers either in health or social care this can be with budgets or identifying a placement in an emergency, during these times I always ensure I prioritise my cases load and work on the urgent cases, also ensuring my other cases are stable.

  • My role within the Transforming Care team involves working within the service finder framework and therefore, I have good experience and knowledge of following what is required of the involved processes and procedures. When supporting the multi-disciplinary teams with a move I have been able to guide others on this knowledge to ensure smooth progress with the planning for the individual.
  • Within my role I work with social workers who are supporting people in hospital who have a forensic history and are on the discharge pathway plan. My role is to explore services that will meet the persons needs and support them live in the community safely and live life independently to the best of their ability.
  • Part of my role is to ensuring services are meeting individual needs, the design and the environment of the service is adaptable for people I am commissioning for, this involves partnership working with professionals and carers.

  • As a Senior Preparing for Adult Practitioner (PFA) within the 0 to 25 together service. My main duties are working with young people from the ages of 14 to 25 years.
  • My role as a preparing for adult practitioner my role involved supporting young people and families with transition and preparation from children to adult services ensuring they have a seamless transition of support, and they have a good understanding and expectation of what will happen when their son/daughter transition to adulthood.
  • I undertake Care Act assessments and Child in Need reviews, the focus of the assessment is putting the young person in the centre, to ensure the young person is involved with the assessment process.
  • For young people and their families accepting changes from children to adult life can be difficult for them emotionally as they are having to adapt to new environments/people and processes. As a practitioner I always ensure information is given in a way the families are able to understand.
  • Working with community resources such schools/college/provider allows the young person to develop their skills and have opportunities within the community/society. I also work alongside connexions and the SEN team, ensuring we all work in the best interest of the young person within education and community support.
  • I manage my own caseload; I ensure I can manage my case load accordingly and ensure I prioritise cases that are more complex than others.
  • Within my role I also undertake mental capacity assessments and best interests’ decisions in line with MCA Act 2005 and Code of Practice 2007, I also work with young people from the age of 14 to 25. I therefore follow two safeguarding policies framework, the Children Act safeguarding board and the adult safeguarding board.

  • I also am aware how essential it is for regular reviews to take place, so records are kept up to date and any issues addressed in a timely and professional manner. I have a good understanding of ACSIS and ICS I have worked on both systems and understand Childrens and adults E Brokerage systems and funding streams.
  • Within my role I always ensure I regularly review care packages as there can be changes to a person needs, such as the service may not be working well for the person or could be over/under provisioned on most occasions the person requires an updated social care/health assessment to reflect their current needs, environment may require adaptation or more involvement from health proffessionals.

  • Within my role I worked with 18- to 25-year-olds with learning disabilities The team consisted of 5 social workers 2 Transition coordinators and 1 Community care officer we covered The North and east Herts of the county and Individuals who were placed out of county.
  • As a community care officer, I supported adults, parents and carers supporting them to access support services, whilst promoting independence and trying to always ensure a person-centred approach.
  • During my time as a CCO I worked alongside social workers with completing assessments and explore services that would meet the individual’s needs, my role was to explore services for the individual such as day opportunities and respite services. Part of my role was to ensure carers have a break from there caring roles and putting them in touch with local groups for support.
  • Woking closely with Send/Connections, schools, and colleges to support individuals leaving education, this helped to identify individuals who would require high level of support due to complexity of support needs.

  • As a Day service organiser, I lead on daily programmes for people with learning disability supporting them to be able to connect to there local community, my role involved ensuring the services available are adapted and accessible for the individuals, supporting individuals to build friendships and have the opportunity to integrate into the community.

  • Directly delivering care for individuals who don’t have the skills or the capacity to follow instructions my role involved supporting individuals with personal care and ensuring medication is administered appropriately, support at mealtimes sand ensuring individuals were safe within the day service setting and the community, any concerns would be raised with the managers such as safeguarding.
  • Communication is key when supporting individuals with additional needs I ensured Information format is changed for the individuals to understand, such as easy-read documents and that advocates or carers are involved with thee scare and support should the person lack capacity.
