Frailty Pharmacist

My current role is as a PCN Pharmacist. This has been with St Helens South PCN for the last 4 years. During this
time I’ve completed the PCPEP and become a NMP. In the last 16 months I've become the Frailty Pharmacist for our PCN Frailty team. My week is split with 3 days in the Frailty team and 2 days in GP Practices.
In my frailty role I help our Frailty ANP Matrons with Prescribing queries and advise with regards to Pan Mersey Formulary, NICE Guidelines, Safety and Monitoring requirements, and ensure cost effective prescribing. One if the main cost savings has been with SIP feeds. This can be stopping supplies when no longer indicated or switching from ready to drink bottles to sachets if suitable. I've also shown them how to use the sample services when initiating so can see if patients like them without any cost to the NHS. The majority of my time within the Frailty team is spent conducting SMR's for housebound patients in their homes. Prior to the visit, Pre SMR work is carried out with the support of a Pharmacy Technician. Depending on the circumstances I may ask our Tech to attend the visit also, and often ask to produce a Medicines Reminder Chart which has proven very popular and beneficial for the patients. After the visit I may need further information, advice, referrals, bloods tests and observations with I arrange with the relevant HCP within the team or GP Practice. The Frailty team has been responsible for supporting surgeries with vaccinating care home and
housebound patients with Covid vaccine. I've pre-screened every patient, route planned, facilitated with training and PGDS, and helped with vaccinations.
When working my 2 days in GP Practices I carry out duties as per DES/IIF (listed previously) and activities required by the practice that is within my field of competence. These includes medication reviews, medication queries from patients and GP's, MHRA safety alerts (documentation and action), Safety audits (recently valproate in males), medication reconciliation/changes from Docman letters, PIS work (SGLT2i and Opioids this year).
Attending MMT monthly meetings and working with MMT on shared projects (eg DOAC switching).

More recently I've completed a Heart Failure course at LJMU which has enabled me to support current HF patients with reviewing and optimising
their treatment, and also arranging relevant tests and referrals for suspected new undiagnosed patients.
As a more experienced Pharmacist within our PCN I often provide advice/support for GP Practice Pharmacy techs, PCN techs and Pharmacists, and
GP's.

Overall responsibility for the safe and effective operation of the Pharmacy. From ordering OTC to dispensary stock, and additional services such as TENS machines and Flu jabs. Delivering NHS services - NHS prescriptions, MURs, NMS, minor aliments, EHC, Palliative Formulary, Vaccines, Private services- ED, Tens machines, travel health, OTC sales. Management/training and development of staff. This included staff new to pharmacy and supporting and developing staff from Pre-reg Pharmacists to Pharmacy Technicians and ACTs. Safety checks- daily log book, weekly cd balance check, quarterly audits. Supporting other stores with both existing services and new initiatives.

For the majority of my time here as Pharmacy Manager, and a Regional role for 6 months. As a Pharmacy Manager I directly managed ~ 10 staff, and as a Regional managed 25 Pharmacy Managers (with a total turnover ~£20
million).
