I am part of the hospital discharge team this is a very fast pace team and my duties are....
To refer discharges on a daily basis, to Care providers , liaising with families, care providers and wards to ensure a smooth
discharge.
Notify SPT , Business support , hospitals and care providers on how discharges are processing.
Making Millbrook,Spa, and Tces referrals when required. I make TCE's referrals for my team members who are unable to
order the equipment as they haven't done their training.
Arrange food parcels with local charities.
Arranging deep cleans with different agencies.
Arranging home visits with fire service
Hospital visits to speak to patients regarding their home and discharge.
Document case notes efficiency.
Complete provider concerns.
Liaise with supervisor and manager.
Working with different agencies in MDT meetings,, TOCH, EHO, Fire brigade, GPs, Peabody, changing pathways..
Accompany Social workers on visits if required
I cover the discharge team ladders, duty calls, and the allocations ladder.
I advocate for service users that need support., enabling them to have their wishes heard, what's in their best interest,
promoting their independence .
Arrange meetings within the hospital with various departments, to make them aware of the tech available to ensure that patients are discharged home safely as per the Care Act.
Assist with organising training within the hospital.

Assisting staff with organising equipment ready for the discharge.
My duties are to ensure that children and staff are provided with a health, balanced and nutritional meal daily. Weekly spending and costing on headcount ratio to ensure that I stay within given budget.
I ensure that all my staff are trained to the highest standards required to
work with the kitchen environment with ongoing weekly and monthly assessments.
I complete timesheets for myself and my staff, invoices that are submitted to the finance department and am responsible for ensuring that all my suppliers meet the school’s standards and needs.
In addition to this I have also started an After School Cookery Club for up to 8 children to educate them on all aspects of healthy eating, including preparing a menu, preparing the food, hygiene and health and safety. This is all prepared in conjunction with the school Education Curriculum utilizing Math’s, English and Science.
