I have completed 6 years of full time postgraduate training in Clinical Haematology in Sri Lanka and have a broad knowledge in Haematology. Currently I am working in a district general Hospital as a Specialty Doctor. I have completed 3 months of Observation period in University College London where I had training in the management of acute leukaemia, MDS, autologous and allogenic hemopoietic stem cell transplantation and CAR-T therapy. I also had training in diagnostic haematology including flow cytometry, cytogenetics, molecular genetic testing leading to an integrated diagnosis and prognostication that determines treatment. I had exposure to clinical trials as well. I have recently completed FRCPath Haematology part 1 and 2. I am willing to go through portfolio pathway for specialty registration. I am looking for a post that can support and build me for the process of Specialty registration through portfolio pathway. I believe my best abilities are adapting to new environments and communicating well with patients.

I do not have regular on-call out of hours duties. We have three SPDs in our department rotating the duties in inpatient setup, chemotherapy unit, day unit and laboratory setup and outpatient clinic setup. rotation happens every four to six months. We have a separate ward for haematology patients where we care for patients with suspected haematological malignancies unwell enough to be admitted for investigations, patients with haematological malignancies who need inpatient chemotherapy and patients with complications of chemotherapy. We also care for patients with non malignant conditions such as severe hemolysis, ITP with bleeding, sickle cell anemia complications. we advise regarding haematological problems of inward patients in other wards. I perform urgent bone marrow biopsies of inward patients while in this role. Complications noted in the chemo and day unit reviewed by the respective SPD. In the laboratory setup I report blood films, bone marrow aspirates and and give advices for the clinical problems that come through my bleep. I have a bone marrow biopsy list each week while in this role. In the clinic setup we provide outpatient follow up for patients with lymphoma, myeloma, acute leukemia, myeloproliferative neoplasms and other general haematology problems. I have continuing responsibility for patients under my care and follow up their symptoms and investigations. Although we don't have separate admission units we admit patients who need inward care , under the care of Oncology or Oncosurgery and we provide shared care. We mainly see patients with MPN, low risk MDS, MDS/MPN in our out patient clinic. Also we manage many general haematology and thrombosis and haemostasis problems such as auto immune haemolytic anaemias, deficiency anaemias, ITP, cancer related thrombosis, anticoagulation in AF in clinic setup and cytopenias, bleeding problems, DIC, platelet refractoriness, massive transfusions and bridging anticoagulation in the ward setup. We get referrals from adult and paediatric oncology units, surgical and medical ICU, onco-surgical units and gynaecological oncology units. I'm the first contact person for these referrals and I seek consultant opinion when necessary. I do a daily ward round to see inward patients referred to us and patients admitted by us. We perform and report about 15- 20 bone marrow aspirations and trephine biopsies a day including initial diagnosis, staging for lymphomas and non haemopoietic malignancies and remission assessment after chemotherapy in both paediatric and adult patients. I and the Registrars examine the bone marrow aspirations, cytochemistry including SBB, PAS and NSE stain and trephine biopsies and immunohistochemistry samples of patients and discuss with consultant haematologists. I guide my junior post graduate trainees to report on bone marrows and help them in difficult cases. I decide the immunophenotyping panels for flowcytometry for the initial diagnosis and help the consultants in interpreting the results. Also I assisst in CD34 enumeration for bone marrow transplant patients and MRD assessment by flowcytometry in B ALL patients. we receive 30-50 blood films daily. I supervise reporting of those by medical officers and difficult cases I report myself. I do interpreting of CSF cytology /cytospin preparations in ALL patients. My centre is a main training centre in Clinical Haematology training programme where both diploma ans MD trainees come for short appointments. I do teaching sessions for haematology trainees as well as other pathology trainees and MLTs. I love teaching my juniors. I was familiar with and reported on the normal, and abnormal microscopic appearance of cells in peripheral blood as well as microscopic appearance of the bone marrow aspiration and trephine biopsy . I have performed more than hundred bone marrow biopsies on adults as well as children. October 2018 - December 2018 at National Blood transfusions Service under Dr.AJMLSBA Jayasekara Consultant Transfusion Physician, NBTS During this period I was familiarise with basics of transfusion serology, pathogenesis of transfusion reactions and investigation, diagnosis, management and prevention of transfusion reactions , preparation and storage of blood components, apheresis, donor section , prevention and management of donor complications.
