Summary

I currently work as a senior Healthcare worker in one one of the busiest care home and also Domiciliary settings where we take care of learning disabilities with complex needs, behaviours that may challenging and almost a hundred residents who we have consistently delivered patient care in line with established plans and protocols, manage the team successfully and escalate appropriately. I am vigilant in recognising changes in patients' conditions and promptly reporting these to Nursing staff. For example, I once noticed a patient becoming increasingly hyperactive and not taking medications administered by nursing staff after conducting the observation and head to toe assessment, I promptly escalated this to the qualified nurse in my bay resulting in timely medical intervention. and saving the life of the patient, I was greatly celebrated for this in my unit. I also participate in the development of an effective team and the development of productive working relationships throughout the care home, domiciliary facility and collaborate with appropriate external bodies( such as GP, specialist clinics, Specialist doctors, district nurses , Antimicrobial team etc to promote seamless and holistic care. I am also great at communicating clearly within the multidisciplinary team, with young people, their families , significant other and their carers
