Making Every Adult Matter Coordinator, supporting adults with multiple complex needs in conjunction with homelessness. Chairing frequent meetings with a large multi-disciplinary team of professionals, I coordinated and managed the MEAM project ensuring outcomes and targets were being met for adults with mental health challenges, contact with the criminal justice system, homelessness and addiction.

Working with adults with disabilities in providing support and routes into employment and volunteering opportunities. Working within different project constraints and ensuring support and advocacy remained while adults with disabilities entered work. Applying for Access to Work funding, holding weekly job clubs, liaising with local partnerships and organisations, writing case studies.

Our team based within the Grateley House Residential School ensured safety and continuous support as well as continual learning for young people with Special Educational Needs. I reported to the Head Mistress and set and audited targets and supported in timetabling support and supported with case conference meetings.

Managing support plans, care plans, service records and facilitating in providing domiciliary and outreach support for people with complex needs.

Operating under the directorate of Lifestyles and Work, my principal task was to oversee the transition of people we support into appropriate Voluntary positions, collaborate on Mencap campaigns, work with other community health services and charities and report on development within my service to colleagues.
