• I work in a fast paced unit in which patients present with a wide variety of acute surgical conditions, giving rise to a diverse range of development and learning opportunities. In here i use clinical judgement and use my nursing skills including admitting, assessing and treating acutely unwell patients.
• I work closely within a multidisciplinary team for consistent communication and continued best care practices.
• Assess, plan and implement individualised nursing care, prioritising patient health and wellbeing.
• Monitor and record patient condition, carrying out and reporting regular observations.
• Administer scheduled medications and therapies intravenously, rectally, orally, subcutaneously and intramuscular injections while overseeing pain management plans.

As a Registered Nurse in this acute medical care setting I supported patients above 65 years with complex needs admitted with different medical conditions and scenarios as Frailty, Dementia/delirium, Falls/poor mobility, Incontinence, Infections, Multimorbidity, Polypharmacy, End of Life among others.

• I was part of a multidisciplinary team that promoted patient's functional independence and safety through patient-centred care
• Monitored and evaluated the patient's response to treatment and amend the plan of care accordingly;
• Made alterations in the management of the patient's conditions in collaboration with the team and the patient in line with agreed pathways and policies, procedures, protocols and guidelines
• Contributed for the patient's comprehensive discharge planning

• As a Registered Nurse in a Nursing Home, some of the main activities and responsibilities included on a daily basis to prescribe and supervise the delivery of excellent standards of care which promoted privacy and dignity, whilst enhancing independence, rehabilitation and personal choice of Residents
• This process of nursing care involved the person with learning disabilities, elderly, respite, step-up and step-down care and end-of-life assuming the importance of cooperation with a multidisciplinary team and promotion of family contact and friends to meet the resident's physical, psychological and social needs

• As an Occupational Health Nurse I was part of a multidisciplinary team that provided care to institutional workers through health promotion and prevention of risks associated to workplace by implementing hygiene, medicine and safety measures. Some of the main activities and responsibilities included:
• Performing and evaluation of programs for prevention of work accidents and occupational or non-professional diseases, making analysis of fatigue, factors of unhealthy, risks and working conditions to the preservation of physical integrity and mental worker
• Training and instruction of employees regarding the use of personal protective equipment
• Performing employees pre-assessments, periodic, dismissal and other standards determined by the institution
• Meet the needs of the employees with pre-existent medical conditions or occupational injuries (including first aid, evaluation of vital signs, collection of blood and urine samples, application of bandages, dressings and administration of oral, topical, intramuscular and intravenous medication)
• Ensure the maintenance, cleaning, maintenance, custody and control of all materials, equipment and workplace
