  • In this role I worked with people who were experiencing a range of severe and acute mental health problems in their own homes as an alternative to hospital admission. This involved providing holistic person-centred care, as well as regular liaison with carers and wider agencies. I provided comprehensive assessments of mental state and risk at both the point of entry to the team, and on an ongoing basis which could then inform care and treatment plans and allow for regular monitoring and evaluation of care. I acted as keyworker for a small group of clients under the Home-Based Treatment Team which involved coordinating their care and working within the parameters of legislation such as the Care Programme Approach (CPA), and also the Mental Health Act (MHA 1983) and Mental Capacity Act (2005). I worked as an active member of the multidisciplinary team and liaised closely with other services and agencies, handing over relevant information and managing any safeguarding issues effectively.
  • Completed Non-medical Prescribing course in 2017, supporting out-of-hours prescribing in absence of medical staff to prevent client deterioration, reduce agitation or distress, and avoid hospital admissions.
  • Part of my role involved acting as the duty worker for the team and screening and triaging referrals from a range of sources including Community Mental Health Teams, Primary Care services and A&E liaison and referring on or signposting to appropriate services as needed. I also acted as a gatekeeper for clients who may have required hospital admission and provided support following discharge for those who had been in hospital.

  • I worked on a mixed sex acute ward for 7 years, progressing from a newly qualified nurse to a clinical sister during this time. I worked with patients with a range of acute mental health problems who frequently presented with significant risk to themselves and others. This required developing excellent assessment skills, and skills in de-escalation and conflict resolution.
  • I worked closely as part of a multidisciplinary team and was required to participate in ward rounds, tribunal hearings and safeguarding meetings in line with the MHA (1983). This required excellent documentation and effective communication skills. I also developed extensive knowledge in the use of psychiatric medications and how to monitor both benefits and adverse effects.
