  • Care plans – As a shift leader any area within this that needed updates, where a young person had attended an appointment for health was to ensure is updated. If they have attended a meeting such as PEP or LAC to ensure this is also updated and brief summary of this.
  • Risk assessments – these to be reviewed by key workers which as a shift leader we were to supervise. However, in times where there may be incidents relating to specific Risks that there is s dates brief update in relation to the risk identified and seen. In times where a risk reduces due to continued improvement then key workers will reduce the risk or after 6 months this will no longer be live.
  • · Behaviour management plan – although the care plan covers behaviour management the BMP will be separate document easy to hand to support in times where behaviours arise and how to support as proactively as possible – will contain more in-depth descriptions of the behaviours / triggers and support strategies to utilize. As behaviours tweak change or new ones occur this need to be added and updates on to relevant plans so most up to date as possible.
  • Monthly Key working packs – all the young people are to be provide with key works suitable for their age and understanding and in line with their care needs. To ensure that these are completed or attempts mad have been evidence – one main key work a month to ensure all areas of needs have been discussed / covered.
  • Weekly reports – these are the feedback reports that the home will share with the child's family or social worker depending on the agreement – these to be sent on a Monday after completion of the previous weeks. Sunday task is to ensure that all reports are completed, and all relevant information is incorporated and detailed
  • Allegations or disclosures – where there may be allegation or disclosure that occur, this will require immediate follow up and reporting therefore need to be documented and shared with the onsite DSL who will then follow up with relevant action or seek support from mangers offsite. The Registered Manager/ RI to be directly emailed and contacted at all times to ensure they are aware of the allegation / disclosure as per the on-call guide.
