I have worked in social care for 14 years. I have excellent knowledge of working with service users of various ages and with those of varying abilities and disabilities. I work really well within a team as well on my own initiative. I have very good IT skills and consistently use microsoft packages and excel. I also use various scheduling platforms, such as; Care planner, Access, People planner, CM2000 and Webroster.

My work is always to a high standard. I have excellent time management and record keeping skills. I am efficient and like to keep everything in order and have excellent customer service skills.

When recruiting for new staff i take in to account their family commitments and any other barriers they may come across to ensure they are able to work the shifts that fit around them and also meets the needs of the service.

I have previously worked closely with people who want to do voluntary work, supporting them to find suitable vacancies, developing their confidence skills and attend informal interviews. I have also been successful in helping people to apply and appeal certain benefits and engage with job centres for future employment.Within my role as practice co-ordinator, I need to ensure that the needs of the service are met and the service users we support have the right quality and consistentcy of care needs. This involves co-ordinating the care packages within the community and for those at Ty Glas Y Dorlan. The support worker schedules are completed on a monthly basis. However, flexability around the scheduling is also crucial to support staff to be able to work part time hours, evenings or weekends and when covering staff annual leave or sickness.

Within my current role, I line manage staff and ensure they are successful in their induction and probationary periods, as well as conduct regular staff supervisions, spot checks and appraisals. I manage performance and return to work interviews. I am involved in many line management roles including the recruitment process and selection for enablement support workers and attendance at core management meetings and panel meetings with other allied professionals for 'extra care accomodation'. I carry out initial assessments with service users before a decsion is made with the core team to provide either therapy support or care and support during their stay at Ty Glas Y Dorlan or when commencing a community care package. From here I will carry out risk assessments in relation to the individuals ability as well as any equipment they may require to support their personal care or mobilty. I will then develop care and support plans with the service user and their families to ensure the right level of support is provided.

I have the responsibility to collate quantitative data for the group manager by processing reports from WCCIS. I provide the our Wellbeing Hub Lead with statistical reports which i collate from Access and WCCIS, as well take responsibility for the admission of service users onto our bed management system.

I am also responsible for keeping the staff training matrix up to date which includes any mandatory or specialist training plus all their supervisions/ performance meetings.

My role also includes liaising with brokerage and social workers to ensure that service users in their own homes are having the right level of care and support and those that are being discharged from hospital are returning home with the right sized package of care.

My role also includes liaising with GP's, CPN's and District Nurses to ensure that service users are having their medical needs attended to. I complete regulations 26, safeguarding to ensure individuals are kept safe. I order medication if needed and cheek it when it arrives to make sure it is correct. i complete action plans to comply with regulation 73. I am on call during the week and weekends and if needed i will go in and cower the service. i believe this shows initiative to the other staff as i am a team player.
