  • I am responsible for assessing, investigating, interpreting, diagnosing, treating, managing and evaluating patients within the PCN. I do this through telephon and face to face ACP clinics. These are of varied cases, acute, chronic, minor and complex.
  • I support and work off the duty list, treating and managing over the phone, seeing face to face or organising onward referrals as appropriate.
  • I review, file and manage pathology results.
  • I review ECGs, acting and managing them appropriately.
  • If needed, I can conduct acute home visits, reviewing, assessing and managing acutely unwell housebound patients. I complete TEP forms, can verify deaths and have discussions regarding end of life management.
